Mafia: The Old Country – Man of Honor is a story DLC released on 14 August 2026. It adds two new chapters set in Sicily in the winter of 1905, in which Enzo helps a young Ennio Salieri, plus new content for Free Ride. This guide covers how to start the DLC, what’s in it, which weapons, knives and Charms it adds and what it costs on its own and in bundles. Everything here applies to PC, PS5 and Xbox Series X/S as of 19 October 2026.
How to start Man of Honor
The DLC requires the base game, Mafia: The Old Country. Once you have both, the update notes list two ways to play the new chapters:
- From the main menu: select the
Man of Honorentry in the main menu to jump straight into the new chapters. The catch: you must have finished Chapter 5 of the main story. - New game: start a new game with the Man of Honor chapters switched on. The new chapters then slot in between the original Chapter 5 and Chapter 6, so you play them as part of the story.
If you haven’t finished The Old Country yet but are past Chapter 5, there’s no need to start over; the main menu route works. If you’re just starting the game, you can pick a new game with the DLC switched on and get the story in one piece. For help with the base game, see our beginner’s guide.
What Man of Honor is about (no spoilers)
The story takes place in Sicily in the winter of 1905. You play as Enzo Favara, who teams up with Cesare to help Ennio Salieri, freshly released from prison. Series fans know Salieri: in Mafia, set in Lost Heaven in 1930, he’s the don of the Salieri family. In Man of Honor you meet him as a younger “man of honor” still in Sicily, before he leaves for America.

What Man of Honor adds to Free Ride
Free Ride is the open play mode that arrived in a free update on 20 November 2025; you start it from the main menu after finishing Chapter 2. Man of Honor adds:
- Salieri’s challenges: Standoffs, Assassinations, Circuit Races, Road Sprints and Time Trials. They’re tougher than the base game’s challenges.
- Salieri’s Jobs: hunting down hidden stashes of loot, delivering valuable cars and stealing trucks loaded with fragile goods. You pick them up at the warehouse in Porto Almaro.
- New collectibles: Wanted posters, stunt jumps and sulfur trucks to destroy.
- New achievements.

Some of the new gear unlocks through play, so don’t expect all of it to be available the moment the DLC installs.

How much Man of Honor costs
On Steam, Man of Honor costs €9.99 (as of 19 October 2026). Buying it separately makes sense if you already own The Old Country in an edition without the DLC, such as the Standard Edition. If you’re buying the game now, the Definitive Edition of The Old Country includes Man of Honor along with the Padrino Pack, Soldato Pack and Gatto Nero Pack and digital extras (artbook and soundtrack for both the base game and Man of Honor).
