Mafia: The Old Country is friendly to newcomers: you do not need to play the earlier games, and 2K designed it primarily with players new to the series in mind. Still, a few things are worth knowing before you start. This guide covers which difficulty to pick, including Classic, how the three knife types and knife duels work, stealth, what to spend Dinari and Charms on, what unlocks when, and which settings make life easier. The tips are based on 2K’s official advice, apply to PC, PS5 and Xbox Series X|S as of 19 October 2026, and contain no spoilers.
What to expect
The Old Country is a linear, story-driven game rather than an open world. Hangar 13 says its structure is closer to Mafia and Mafia II: you follow Enzo Favara chapter by chapter as he gets involved with the Torrisi family in Sicily in 1904. The main story takes roughly 13 hours according to PCGamesN and PC Gamer. Veterans of the series will spot nods to the later timeline, which our guide to the Mafia games order lays out.
Which difficulty to choose
There are four difficulty levels. Classic arrived with the Free Ride update on 20 November 2025 and draws on the original Mafia and Mafia: Definitive Edition.

| Difficulty | What it changes | Who it suits |
|---|---|---|
| Easy | your weapons deal more damage, enemies aim worse, more resources from loot, cars take less damage | players here for the story |
| Medium | standard settings | most players |
| Hard | less damage, tougher and more accurate enemies, fewer resources, realistic car damage | experienced action players |
| Classic | less damage, enemies hit even harder, no auto-reload and unspent rounds are lost on reload, limited health regeneration, no aim assist or driving assists, realistic car damage, fewer resources | series veterans |
If third-person action games are new to you, start on Medium. Save Classic for a second playthrough or for Free Ride, where you can use it too.
Knives: Scannaturi, Rasolu and Stiletto
The knife is your core tool in The Old Country. Knives unlock in chapter 2, you can only carry one at a time, and you buy them from Pasquale or Valerio. You can equip them in their shops or at weapon crates.

| Type | What sets it apart |
|---|---|
| Scannaturi | can be thrown (silent ranged kill), lower durability |
| Rasolu | one-hit kill in active combat |
| Stiletto | highest durability |
Knife durability is used up by special actions: picking locks, quick silent kills, throw kills with the Scannaturi and one-hit kills with the Rasolu. You restore it with whetstones found in the world. Your basic knife attack always works, whatever the durability. Some knives also come with perks, such as healing after a takedown.
Knife duels
In a knife fight, listen for the audio cues that signal the next attack. A red flash means an unblockable attack, so you have to dodge. Dodging, parrying and every other input has its own timing window, so button mashing will not help. If your opponent reads your style, mix up your tactics. The game teaches duels gradually through tutorials and feedback during the fight.
Stealth and Instinct
Stealth is mostly optional, though some sections require it. These help:
- Instinct reveals enemies through obstacles.
- Hide bodies. Guards are alerted when they find one.
- Throw coins or bottles to lure guards away.
- A thrown Scannaturi kills silently at range. Remember to pick the knife back up.
Charms, Dinari and shops
Charms grant passive bonuses. You slot them into Enzo’s rosary on the Charms screen, and you can buy extra slots at Pasquale’s shop. You earn Dinari from enemy loot, lockboxes, the story and Free Ride challenges. Spend them as you go on weapons, knives, clothing, cars, horses and their styles, with shops also at the wardrobe, garage, stable and weapon locker. Dinari cannot be bought with real money, as the game has no microtransactions.
Cars differ in weight, power, top speed, gearbox (2 to 4 gears), acceleration, brakes and grip, each rated on a 1 to 5 scale. For horses, keep an eye on stats such as stamina, which sets how many times you can spur them on.
What unlocks when
| Chapter | What unlocks |
|---|---|
| 1 | wardrobe and Charms |
| 2 | knives (DLC knives after meeting Pasquale) |
| 4 | stable with horses |
| 5 | garage with cars and weapon lockers |
Unlocked items are saved to your profile, so you keep them in a new playthrough, in Chapter Replay and in Free Ride. Since the 1 September 2026 update, previously unlocked outfits, hats and hairstyles are also available when you start a new game.
Settings that help
Navigation SignageunderSettings > Gameplay: roadside signs point you to your objective based on where you are. It is on by default, so leave it that way.- Customizable Aim Assist, manual transmission, Skip Drive & Ride, simulation driving, camera FOV and camera shake can all be tuned to taste.
- Cover: choose between Persistent Crouch and the Classic Mafia style.
- Accessibility: subtitle size, colorblind modes, toggle or hold to sprint, and Simplified Struggle, which completes rapid button presses for you.

Collectibles
Around the world you collect notes, newspapers, saint cards and photographs. Some of them give rewards: Trinacria you can exchange for Dinari, or Charms. There is also a mysterious type of hidden item for you to discover yourself.
Free Ride, Cinema Siciliano and Photo Mode
Free Ride arrived as a free update on 20 November 2025 and replaced the earlier Explore mode. You launch it from the main menu after finishing chapter 2, and it does not change the main story. It offers racing challenges with Pasquale and Gaspare (Circuit Race, Road Sprint, Time Trial, Horse Race) and combat challenges with Valerio (Standoff, Assassination). Each challenge has three reward tiers paid in Dinari, which makes them a good source of money.
The same update added first-person driving, Photo Mode and Cinema Siciliano: Sicilian voice-over, a black-and-white film look, old film reel sound and a minimal HUD. You can open Photo Mode any time outside cutscenes via the pause menu and Options, and set a shortcut under Settings > Controls > Photo Mode Shortcut.
Once you finish the story, the Man of Honor expansion is waiting. What it costs and which edition includes it is in our editions and price guide.

Listen for the audio cues and dodge when you see a red flash, because that attack cannot be blocked. Every action has its own timing window, so button mashing will not work.
